St Clements Partnership uses a record called a Summary Care record (SCR) which is an electronic record of important patient information, created from GP medical records. It can be seen and used by other authorised NHS staff in other areas of the health and care system involved in the patient's direct care. As a minimum, the record consists of:
· Current medication.
· Allergies and details of any previous reactions to medications.
· Name, address, date of birther and NHS number.
